Iowa uses a provider choice EVV model in which all MCOs have selected CareBridge Health as the state EVV solution and data aggregator. Providers may use an alternative EVV vendor of their choice, but that vendor must be CMS-compliant and must aggregate visit data through CareBridge for claim submission. CareBridge supports multiple verification modalities including mobile app, Interactive Voice Response (IVR/telephonic), and Fixed Object Device (FOB) for caregivers without smartphones. Personal care services (PCS) required EVV by January 1, 2021; home health care services (HHCS) by January 1, 2024.
What is EVV?
Electronic Visit Verification is the federal requirement (under the 21st Century Cures Act §12006) that personal-care and home-health visits paid by Medicaid be electronically captured at the point of care — who provided the service, who received it, where, when it started, and when it ended. CMS pushed enforcement to the states, which is why the day-to-day experience varies so much.
What "state-offered with opt-out (choice model)" means in Iowa
The state offers a default aggregator (typically free) and lets providers opt out for a third-party vendor. The third-party path may require certification or interface fees.

In open-model states, our visits are EVV-compliant on their own.
